APPOINTMENTS: All appointments must be scheduled in advance. Email to lavena@wilkesnp.com
for appointment options. New patient requests must be initiated by submitting the form available on the website Wilkesnp.com. Notify the office 24hrs in advance if you are unable to keep a scheduled appointment to avoid fee and allow for timely rescheduling.
Telehealth: Initial evaluations and follow up appointments may be conducted virtually. You must live in S.C. to be eligible for this service. You will need access to a computer with good internet connection in location that allows for privacy.
In Person: Appointments held in person are conducted at 222 Adley Way, Greenville, SC 29607 (Sterlingworth Center). Please minimize time spent in waiting room by entering building just prior to appointment. Maintenance of stable medication prescribing generally requires quarterly appointments. Periods of destabilization and/or need for monitoring adjustments may require more frequent appointment intervals.
Upon a period of 6 months lapse between appointments (unless prior approval obtained) services will be placed on inactive status until rescheduled under the new patient protocol.

COMMUNICATION: Due to limitations on direct phone contact availability, e-mail contact is preferred. E-mail allows for more timely responses to inquiries. Please allow 24hrs for a response before resubmitting any request. If you have an urgent need or emergency, you must call 911, go to nearest ER/ Urgent Care or contact primary care provider.

MEDICATION To avoid interruption in your treatment, please assure that you are scheduled to be seen prior to need for medication refills. Most medications should not be stopped abruptly and may result in unnecessary discomfort or potential serious effects. Afterhours or between appointment prescriptions cannot be relied on to maintain continuous treatment. Please plan and schedule accordingly
Controlled substance prescriptions: You may be required to sign a separate consent/contract to be prescribed any controlled substance (i.e.: benzodiazepine for anxiety/insomnia and stimulants for ADHD). You are responsible for securing these medications from loss, take them only as prescribed and not to share with others. These medication prescriptions are to be received as part of scheduled appointments. They are not to be requested for early refill due to loss of Rx or “running out early”. Drug screening may be required without prior notice. Misuse may result of termination of services.


PAYMENT/FEES: No insurance claims are filed through this office. You may submit a receipt for services to your insurer if out-of-network benefits are available. Credit/Debit, Check or Cash accepted.
Fees are due at time of service unless alternative arrangements have been made in advance.
New Patient Appointment (approx. 75mins): $245               New Patient No Show fee: $100
Medication Follow-Up (approx. 20mins):  $95                 Medication/Therapy Follow-Up (approx. 40mins): $145
Late cancellation (less than 24 hrs. notice): $25          Forms/Letters: $25 min. (Paid in advance of completion)

Follow up No Show: $50 (note: 2 no-shows may result in discharge from service)
